DD FORM 254 (13 CON’T.), MAY 2010

Visit authorization requests (VAR) are to be sent to USTDA Office of Administration who will then forward completed VARs to DoS, DS/IS/IND via facsimile at 571-345-3000 or email to DS_IND_contractorVARS@state.gov. Termination VARs are required for contractor personnel removed from contract performance for any reason including contract termination or expiration. A VAR is valid for 12 months and shall be renewed 30 days prior to expiration.

All DD Forms 254 for subcontractors shall be forwarded to the CO for certification prior to issuance to the proposed subcontractor, and prior to subcontractor personnel gaining access to any classified information or controlled access areas.

While at USTDA location which is serviced by DoS, the contractor shall comply with applicable DoS regulations relative to the protection of classified and/or sensitive information, including the National Industrial Security Program Operating Manual (NISPOM) and 12 Foreign Affairs Manual (FAM) 500 and 600 as applicable. DS/IS/IND is responsible for inspecting contractors assigned to USTDA locations.

Contractor shall immediately report any adverse information concerning any cleared contractor personnel performing on this contract to the CO and Contracting Officer Representative (COR) who will in-turn notify the USTDA’s security officer; DSS in accordance with the NISPOM; and DS/IS/IND in accordance with 12 FAM 576.

Contractor personnel working on this contract must report impending marriage, cohabitation, and other continuing bonds of affection with foreign nationals to the CO and COR, who will in-turn notify DS/IS/IND in accordance with 12 FAM 275.

Classified storage is not a requirement for performance on this contract; therefore, no discussion, generation or storage of classified information shall take place at the contractor’s facility.

13: Contractor personnel assigned to this contract shall possess SECRET personnel security clearances issued by Defense Security Service (DSS) for contract performance. For contractor personnel awaiting the issuance of an interim or Final Secret clearance, the Contract Officer (CO), at their discretion, may allow for contract performance until such time as the interim, or final Secret clearance is awarded by DSS. Uncleared contractors shall be processed for, and receive a Department of State (DoS) Personal ID Facility Access Card (FAC) prior to contract performance. Uncleared contractor personnel are not eligible for access to classified information.
