About this file

This document contains templates and instructions for a past performance questionnaire related to a federal solicitation for one-time CNC health check and preventative maintenance assessments. The solicitation is being conducted by the Department of the Air Force Materiel Command Air Force Sustainment Center as a FAR Part 12 & 13 competitive acquisition using past performance and price as evaluation factors. Interested vendors are instructed to send the attached questionnaire to points of contact on relevant past efforts for completion. Completed questionnaires must be returned directly to the government, not the vendor, by the specified due date.
