About this file

This document is a Request for Substitution/Exemption form issued by the City of Phoenix Public Transit Department. The form is designed to allow prime contractors to request changes to Disadvantaged Business Enterprise (DBE) subcontractor utilization, including releasing, substituting, changing the scope of work, or reducing the amount proposed for a DBE subcontractor. The form must be submitted to the Public Transit Department Compliance Section at ptdcompliance@phoenix.gov for approval prior to making any modifications to the proposed DBE utilization statement or monthly DBE utilization form.

The form requires the prime contractor to provide detailed information about the proposed changes, including the contract number, project details, affected DBE subcontractors, and the specific type of exemption being requested. The contractor must also summarize their good faith efforts in finding additional DBE participation and attach supporting documentation. The form includes sections for the contractor's authorized representative to sign and provide contact information, as well as a section for the Contract Compliance Specialist to review and approve or deny the request, potentially revising the DBE goal and utilization percentage.
