About this file
This is a Request for Substitution/Exemption form issued by the City of Phoenix Public Transit Department for use in managing Disadvantaged Business Enterprise (DBE) participation on state and local contracts. The form must be submitted to the City of Phoenix Public Transit Department Compliance Section (ptdcompliance@phoenix.gov) prior to making any changes to proposed DBE utilization statements. Prime contractors may request exemptions to release proposed DBE subcontractors, substitute DBE subcontractors, change the scope of work performed by DBE subcontractors, or reduce amounts originally proposed for DBE subcontractors. The form requires the authorized representative of the prime contractor to certify their authority and provide a summary of the exemption request along with documentation of good faith efforts made to secure additional DBE participation. The form includes a verification section where the contractor must indicate approval status and include supporting documentation such as the Initial SBPP Plan, DBE Annual SBPP Plan, and certification of final payment. The compliance section of the City reviews and approves or denies requests, noting the revised DBE goal and utilization percentages, and assigns a Contract Compliance Specialist to manage the submission.
This document serves as the procedural mechanism for the Heat Relief Supply Distribution Management contract (GGS-26-0100) in Phoenix, Arizona, which specifically requires DBE and small business participation under the United States Department of Transportation's DBE program. Contractors are obligated to demonstrate good faith efforts to promote small business and DBE firm participation through outreach activities and reasonable utilization of eligible business services. Monthly documentation of DBE and small business participation must be provided through the City's Certification and Compliance System, with contractors required to identify potential goods and services categories available from DBE-certified suppliers. The contract structure includes insurance requirements of $2,000,000 in general liability, $1,000,000 in automobile liability, and workers' compensation coverage, with compensation based on a unit hourly rate for a Heat Relief Supply Distribution Manager at an estimated 5,000 hours annually for a five-year term beginning February 1, 2026.
