About this file

This is an employee healthcare expenses certification form required by the State of Maryland under State Finance & Procurement Article Title 17, Subtitle 8, applicable to bidders and subcontractors on state and local contract opportunities including the SGHC Snow Removal Services 2026-2027 project at Spring Grove Hospital. The certification requires bidders or subcontractors to declare compliance with Maryland's employee healthcare expense requirements for all employees who will work on or at the project site. No specific project timelines, quantities, site visits, or bidder meetings are detailed within this certification document.

Bidders must certify they meet one of three healthcare expense options: paying aggregate employee healthcare expenses of at least 5% of aggregate social security wages paid, paying 50% or more of the required premium for credible health insurance plan coverage, or being exempt due to having 30 or fewer employees. A credible health insurance plan is defined as group coverage written or administered by disability insurers, health care service plans, fraternal benefits societies, self-insured employer plans, or similar entities in Maryland or elsewhere that provide medical, hospital, and surgical coverage. Falsification of this certification carries civil penalties ranging from $2,500 to $25,000 per violation under Maryland Annotated Code Section 17-804.
