About this file

This is an attachment form requiring bidders to list their current or prior state contracts with the State of Maryland. The form is designed to evaluate a bidder's experience and past performance for an Invitation for Bid (IFB) issued by the Maryland Department of Information Technology (DoIT) for Hive IQ licensing and support services, a secure collaboration and incident response platform. Bidders must provide comprehensive information for all contracts performed with the State of Maryland within the past five years, including the state contracting entity, description of goods and services provided, contract dollar value, contract term dates, the name and contact information of the state employee who directly oversaw the contract, and whether the contract was terminated early or any renewal options were not exercised. The procurement officer will use the information gathered regarding past performance on state contracts to determine the bidder's responsibility and consider it as part of the experience and past performance evaluation criteria for this solicitation.

The form itself does not specify pricing terms, set asides, funding requirements, or other financial details, as it functions solely as a reference document for bidders to document their contract history. The form provides blank template sections for bidders to complete information for multiple state contracts, with specific fields for state contracting entity details, employee contact information, contract titles and numbers, contract values, contract performance dates, and detailed descriptions of goods or services provided. This attachment represents a standard responsibility and experience evaluation tool used in Maryland state procurement processes.
