About this file
This is a vendor questionnaire attachment for RFP #2026-4 issued by Polk School District for commercial food supply and distribution services to support nutrition services across ten school sites. The questionnaire requires proposers to demonstrate qualifications, capacity, and operational capabilities through detailed responses addressing company background, K-12 food service experience, and relevant references. Vendors must describe their ordering processes, including web-based accessibility and lead time requirements, as well as their ability to provide nutritional information, allergen documentation, CN labels, food safety documentation, and recall notification procedures. The document establishes expectations for order protection and allocation controls, Buy American compliance support, new product introduction timelines, out-of-stock communication protocols, and real-time reporting capabilities.
The questionnaire further requires vendors to detail delivery proposals for the ten sites, specifying delivery frequency, time windows, temperature control procedures, and driver communication protocols, along with processes for resolving invoice discrepancies and supporting USDA Foods utilization. Proposers must submit comprehensive startup and transition plans demonstrating pre-service planning, staff training, and operational readiness timelines, along with anticipated challenges and mitigation strategies. Customer service requirements include specified hours of operation, after-hours support availability, escalation procedures, average response times, dedicated account management structure, and value-added services such as menu planning assistance, product demonstrations, culinary support, and farm-to-school initiative assistance. The questionnaire serves as a critical evaluation tool to assess vendor suitability for providing reliable, compliant food supply and distribution services to Polk School District.
