About this file
This is a Past Performance Questionnaire template designed for vendors responding to the Library of Congress solicitation for Braille and Tactile Embossing Services. The questionnaire serves as an evaluation tool to assess the past performance capabilities and experience of offerors bidding on this simplified acquisition contract.
The questionnaire is intended to gather information about potential contractors' experience with specialized braille services, including document transcription into braille, production of eBraille (.brf) files, and embossing and binding of documents. As an appendix to the solicitation, this template does not contain specific performance metrics, response dates, or pricing terms, but rather functions as a standardized form that offerors must complete to demonstrate their qualifications for performing the National Library Service's braille and tactile graphics production requirements. Contractors are expected to provide detailed responses about their relevant experience with similar projects to support their proposals for this 5-year IDIQ contract being offered by the Library of Congress.
