About this file

This is a Qualification Application form for community-based organizations (CBOs) interested in partnering with the California Department of Public Health (DPH) to provide sexually transmitted disease (STD) and hepatitis C virus (HCV) testing services and related public health activities. Applicants must provide comprehensive organizational information including business structure, years of operation, facility details, and staffing contacts. The application requires detailed documentation of experience conducting health-related needs assessments, STD and HCV testing programs, community engagement and collaborative efforts, data analysis capabilities, and any history of formal grievances or complaints. Organizations must also outline their capacity to accommodate public health staff needs, including workspace and electronic health record access, and specify their utilization plans for in-kind staffing support provided through the partnership.

The application further requires applicants to detail their current testing methodologies, service expansion plans, populations served, and monthly service volumes across specified demographic groups including young adults, women of childbearing age, pregnant persons, LGBTQ+ individuals, intravenous drug users, unhoused persons, and recently incarcerated individuals. Applicants must enumerate prevention materials and services currently offered such as PrEP, condoms, harm reduction services, and counseling. The form requests quantitative data on HIV, syphilis, chlamydia, gonorrhea, and HCV tests performed in the preceding twelve months and projections for the coming year, with applicants required to explain their rationale for projected testing volumes. The application serves as the primary evaluation instrument for determining organizational eligibility and capacity to deliver integrated STD and HCV testing and prevention services to high-risk populations.
