About this file

This is a Past Performance Questionnaire template for RFQ #95332426Q0024 for Legal Advisory Services. The document is designed to collect references from past clients or government representatives regarding an offeror's performance on previous contracts.

The questionnaire requests information about a specific reference contract, including the contract number, total contract value, period of performance, and whether the offeror served as prime or subcontractor. Evaluators must rate contractor performance across seven key assessment elements using a five-point scale: Very Good (VG), Satisfactory (S), Marginal (M), Unacceptable (U), and Not Applicable (NA). The assessment elements are: (1) Cost/Price Control—ability to manage costs and deliver at agreed-to prices; (2) Schedule Adherence—compliance with delivery schedules and timely submission of deliverables; (3) Performance—compliance with contract requirements and statement of work; (4) Quality—adherence to quality assurance and performance-based contracting requirements; (5) Business Relations—cooperative behavior, personnel stability, and responsiveness to changes; (6) Personnel Qualifications—ability to staff and retain qualified personnel; and (7) Overall Rating. The questionnaire also requires narrative responses identifying contractor strengths and weaknesses, and asks whether the reference would award to the contractor again. A certification section requires the reference to attest to the accuracy of ratings provided.
