Quoters must identify their business size classification for NAICS code 622210 as registered in SAM and ensure this NAICS code is added to their SAM registration if not already present. The template mandates that quoters submit a signed SF1449 form and any issued Solicitation Amendments (SF30) signed by an authorized representative. Quoters must confirm they have reviewed insurance requirements in Section H.13 and commit to providing a Certificate of Insurance meeting or exceeding those requirements within 10 days of purchase order award, with a note that post-award waiver requests will not be considered. Additionally, pricing quotes are valid for 120 calendar days from submission date, and quoters must disclose any assumptions, exceptions, or conditions in a separate attachment. All certifications and statements require signatures from individuals authorized to bind the quoter.
