About this file
This document is a Quality Assurance Surveillance Plan (QASP) Matrix for the Above Ground / Underground Storage Tank (AST/UST) Safety Manager Course. The matrix details specific performance requirements, measurement metrics, and surveillance procedures for key deliverables, including student attendance sheets, course completion reports, student critiques, key personnel management, audits, and invoicing.
The QASP establishes strict performance standards with 0% maximum error rates for most deliverables, requiring contractors to submit documents like sanitized student attendance sheets within four business days of course completion and comprehensive course completion reports within 30 days. Routine inspections will be conducted, and failure to meet requirements will result in details being recorded on a "past performance report card". Additional requirements include maintaining less than 20% key personnel turnover, encouraging student critique participation, and submitting accurate invoices within ten business days after travel, with potential increased government oversight for non-compliance.
