About this file

This document is a Quality Assurance Surveillance Plan (QASP) Matrix for Incident Command System (ICS) and Oil & Hazardous Substance Tabletop Exercise (OHSTTX) Courses. The matrix outlines specific performance requirements, measurement metrics, and surveillance methods for key contract deliverables, including student attendance sheets, course completion reports, student critiques, key personnel management, audits, and invoicing. Each requirement has defined performance standards with a maximum error rate of 0-5%, to be monitored through routine inspection, with potential consequences of recording details on a "past performance report card" if standards are not met.

The matrix includes specific timeliness and accuracy requirements such as emailing sanitized student attendance sheets within four business days of course completion, submitting course completion reports within 30 days after each class, maintaining less than 20% key personnel turnover, and processing invoices within ten business days after travel. The document also includes a compliance requirement for FAR 52.222-50 related to Combating Trafficking in Persons, which requires 100% compliance and is subject to potential government remedies if violated.
