About this file
This is a Single Source Justification document for a simplified acquisition procedure (SAP) from the Department of Veterans Affairs Network Contracting Office 10 in Indianapolis, IN. The document justifies a sole-source award to Skytron for the repair and repurposing of operating room boom lights, following deficiencies found during annual planned maintenance. The estimated cost is $26,420.50.
The justification explains that Skytron must perform the repairs as they are the original equipment manufacturer and conduct the annual maintenance on the boom lights. The work requires Skytron-certified technicians to maintain warranty coverage and ensure electrical and mechanical safety. While market research was not conducted, the RFQ will be posted as required, allowing other qualified companies to submit quotations. The document is signed by Contracting Officer Luke A. Turner and references acquisition plan ID 36C250-25-AP-1312 and 2237 number 583-25-2-663-0039.

VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT
Revision: 01 Effective Date: 11/15/2024 Page 1 of 1 Acquisition ID#: 36C250-25-AP-1312
DEPARTMENT OF VETERANS AFFAIRS
Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)
Acquisition Plan Action ID: 36C250-25-AP-1312
1. Contracting Activity: Department of Veterans Affairs, Network Contracting Office 10 (NCO 10), 8888 Keystone Crossing Drive, Suite 325, Indianapolis, IN 46240. 2237: 583-25-2-663-0039.
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
Deficiencies were found during the completion of the annual planned maintenance on the booms in the operating rooms. The booms will be repurposed and repaired to meet standards that will allow them to remain operable. Total cost will be $26,420.50.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Skytron performs the annual maintenance on the boom lights and are able to install the necessary replacement original equipment parts to ensure proper fitment and reliability. Skytron requires all corrective and preventative maintenance to be completed by trained and certified technicians to prevent damage and additional costly repairs that could result in unnecessary downtime. The OR Boom lights must be repaired from Skytron to maintain proper warranty and ensure the electrical and mechanical safety of the lights are not compromised.
4. Description of market research conducted and results or statement why it was not conducted:
Additional market research was not done. The RFQ will be posted as required. Any additional companies claiming the ability to provide these services will be able to provide a quotation.
5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
