STATEMENT OF WORK

Project # 652-18-930

RENOVATE EMERGENCY DEPARTMENT HEADWALL

AT

Hunter Holmes McGuire VA Medical Center Richmond, VA

1.0 INTRODUCTION

1.1 SCOPE

The contractor shall supply all labor, materials and supervision necessary to reconfigure the emergency department at Hunter Holmes McGuire Hospital main building 500 located at 1201 Broad Rock Blvd. Richmond, VA 23249 in accordance with the drawings and specifications. Contractor must comply with the current VA standards for hospital construction and patient care. This project shall be completed in phases. Project shall be phased based on the hospital needs of how many rooms could be down at a time, phases shall be no more than 2 rooms at a time. Each phase shall be fully complete and punched out before the next phase begins. The start of each Phase shall be coordinated with the Richmond hospital COR. Work shall be conducted during normal business hours 8a-4:30p.

Special provisions should be made for afterhours work specifically for operations producing noise, shutdowns or when work occurs that impacts critical areas of the medical center (examples include:

OR, Lab, Mental Health, Inpatient Units, etc.), or as otherwise outlined in the bid documents. In these cases, work shall be performed after hours and shall be coordinated with medical center staff through the COR. Any shutdown dealing with med gas can’t last for more than 8hrs. and times must be coordinated with the Emergency department staff. Before any shutdown request, the contractor must notify the COR via email two weeks prior to the shutdown. Every time contractors shuts off and turns back on the med gas valves, they must have a med gas certifier here on site and the signed certified form should be provided to the COR. Contractor shall incorporate and maintain all provisions of ICRA’s and ILSM’s.

1.2 BASIC REQUIRMENTS

Overall requirements include the efficient management of the Contract, but not limited to, the submission of accurate, on time, and quality contract deliverables, as well as, timely identification and solution of impediments to successful project execution. Technical requirements include, but are not limited to, early involvement in the process to allow for the development of the most cost-effective and technically sound approach or solution. The Contractor shall perform all work in accordance with federal, state, and local codes, statutes and regulations.

2.0 GENERAL REQUIRMENTS

Contractor shall store all materials offsite and only bring materials to be installed the same day to the project site. Contractor shall coordinate delivery, storage, and handling of all materials with the COR to designated prepared areas to minimize site storage time, impact to the hospital, and potential damage to stored materials. Contractor is responsible for removing, storing and reinstalling any wall mounted items.

Work shall include, but is not limited to demolition, architectural, electrical, (data cabling), plumbing, structural, and mechanical improvements. All contractors shall complete safety training with Richmond VAMC Safety team prior to work beginning. Contractor must comply with all requirements of VA Specification 01 35 26 Safety Requirements. Refer to attachment for specs.

3.0 SPECIFICS

Contractor shall provide and install a head wall unit and bed locators in all 18 patient rooms specified on drawings. Contractor must relocate and fit all wall mounted existing and new headwall units, including but not limited to data, electrical, oxygen, vacuum and other medical gases in a compact package on the head walls. This must be done cleanly, reducing both visual clutter and trip hazards.

Outlets and openings should be strategic and economic, minimizing the need to stoop or bend to access them. Contractor must have a med gas certifier on site for every med gas shutdown and testing. Head walls must be submitted to the COR or VA interior designer for approval prior to procurement. Head walls are to match hospital standard and similar to others around the facility.

Below is detail description of all the devices and equipment’s, that are located on the headwall wall, in every room that is shown on the drawings. The contractor must do field verification prior to doing any construction or work on site.

All wall units taking down for construction by contractor must be reinstalled after head walls are installed. Contractor must test and make sure every unit installed or reinstalled works properly before moving onto the next rooms.

4.0 PERIOD OF PERFORMANCE AND HOURS OF WORK

Period of Performance: 200 days after NTP has been issued (after award). Contractor will coordinate final day and time with VA COR before schedule to do work.

4.1 HOURS OF WORK

The normal hours of work on installations are from 8:00 a.m. to 4:30 p.m., Monday through Friday, excluding holidays. Construction hours of work and access to work sites are from 7:30 a.m. to 4:30 p.m. For work during other than these hours of work, the Contractor shall submit, in writing, for the

Contracting Officer's approval, a notice of any period of scheduled work other than the normal hours of work specified above. This notice shall be submitted not less than two weeks prior to each period of work scheduled at times other than normal hours of work including holidays.

US Government holidays. When any of the US Government Holidays fall on a non-work day, the holiday will be observed on either the preceding or succeeding workday. The choice of any such preceding or succeeding workday shall be coordinated with the COR and the CO.

5.0 WAST MANAGEMENT

Contractor shall coordinate with COR for waste removal. All waste shall be removed at the end of each day. Contractor shall remove and dispose of waste in accordance with all legal and VA Specification 01 74 19 Construction Waste Management.

6.0 UTILITIES

Contractor shall protect all existing equipment, utilities, structures, etc. to remain. When the space is turned over to the contractor, the contractor will be responsible for repairing all damage within the construction site. If any damage is noticed prior to starting work that may be misconstrued as damage caused by the contractor, the COR shall be notified immediately, and the damage documented as existing prior to beginning demolition.

7.0 SAFETY BARRICADES AND WARNING SIGNS

The Contractor shall furnish, install, and maintain adequate barricades, warning signs, and flares for indoor and outdoor hazards related to construction in accordance with contract clause 52.236-13, Accident Prevention. Additionally, outdoor barricades shall follow base standards. Outdoor barricades for hazards including construction site fence/barrier shall be submitted for government approval to the CO and/or COR. Government review will include VA safety, VA police Services, and Engineering Office review. Contractor is responsible for maintaining building life safety standards prescribed through an Interim Life Safety Measure risk assessment permit requirements issued prior to any work beginning. All contractors are required to be safety trained by VA Safety department prior to commencement of work. Contractors are also required to minimally have an OSHA 10 Hour certificate within the last two years.

7.1 USE OF ROADWAYS

For hauling, use only established public roads and roads on Medical Center property and, when authorized by the Resident Engineer, such temporary roads which are necessary in the performance of contract work. Temporary roads shall be constructed by the Contractor at Contractor's expense. When necessary to cross curbing, sidewalks, or similar construction, they must be protected by well-constructed bridges.

8.0 INSPECTION

8.1 PRE-FINAL INSPECTION

The Contractor shall conduct a pre-final walk-through inspection and publish the pre-final inspection findings in a pre-final inspection report in accordance with the format specified at the pre-construction conference and defined in the Contract.

8.2 FINAL INSPECTION

The Contractor shall conduct a final acceptance inspection and publish the findings in a final inspection report in accordance with the format specified at the pre-construction conference and defined in this Contract. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final report. The final inspection report shall certify that all items of the design have been implemented and that the construction is complete. At the final inspection, the Contractor shall present a Certificate of Occupancy to the COR to coordinate for signature and acceptance.

9.0 NOTIFICATION REQUIRMENTS

The Contractor is required to notify the CO and COR of critical issues that may affect the contract performance and/or human health and the environment. The types of issues that require notification include, but are not limited to, health risks, spills, unexpected utility crossings, unusual weather conditions, unacceptable materials, changes in critical personnel, unforeseen hazardous material. On critical issues, verbal notification shall be made immediately followed by written notification as soon as practical. For critical issues pertaining to human health and the environment, the Contractor shall immediately notify the CO and stop work. Work shall only resume at the direction of the CO following resolution of the issue.

10.0 DE-MOBILIZATION

The Contractor shall remove temporary facilities and implement erosion control measures such as seeding, mulch, sodding, and erosion control fabrics; restore roads, structures, and utilities; and plant trees, shrubbery, grasses, and other vegetation as required in the Contract. The Contractor shall document and report on activities and train on-site personnel to perform required maintenance as specified in Annex D (Submittal Procedures). The Contractor shall decontaminate equipment and facilities, decommission facilities as necessary, and fully restore the site.

11.0 OTHER CONTRACT REQUIRMENTS

11.1 SHIPPING AND SERVICES

The Contractor shall provide transportation for construction material and equipment to the Asheville VA Medical Center and for movement of the materiel and equipment from its Point of Delivery to the ultimate in-country destinations. The Contractor shall self-insure such shipments or obtain commercial insurance without any right of subrogation of any claim against the United States Government.

11.2 TRAVEL

All United States Government Contractor personnel shall be in economy class or as otherwise directed in the United States Joint Federal Travel Regulations.

11.3 NICOTINE FREE CAMPUS EFFECTIVE 1 OCT 2019

As part of the U.S. Department of Veterans Affairs’ (VA) commitment to provide excellent health care for Veterans, the department will implement a new policy restricting smoking by patients, visitors, volunteers, contractors and vendors at its health care facilities by October. VHA’s new smoke-free policy applies to cigarettes, cigars, pipes, any other combustion of tobacco and non- Federal Drug Administration approved electronic nicotine delivery systems (ENDS), including but not limited to electronic or e-cigarettes, vape pens or e-cigars.

11.4 COMPLIANCE WITH EMERGENCY MANAGEMENT DIRECTIVES

Contractors shall comply with all safety directives and emergency management instructions as directed by the local VA Medical Center site specific policies & procedures, Local Construction Handbook, and any subsequent updates to the handbook directed by policy or law. These directives are to ensure preparedness and safety of all personnel doing business on the VA Medical Center property. Notifications of “Exercise” and Real-World events are typically made via a Public Address/Giant Voice Systems. The contractor shall responsible for the participation of their on-site employees and reasonably respond to these announcements in the safest means practicable.

All employees of the contract are required to participate in mandatory required facility exercises to meet facility accreditation and compliance. The timeframe for these exercises are typically short.

Examples include, but not limited, Active Shooter/Active Threat, Tornado, Bomb Threat and Fire Drills. The contractor/subcontractor employees shall be required to respond to any VA Medical Center Drill as if it were a real-world emergency/threat.

All employees of the contractor/subcontractor shall participate in preparations for all VAMC Safety or Emergency Management as directed. All hazards, events, and disasters including inclement weather, earthquakes, mandatory evacuations and also includes site safety and preparations for visits by high profile VIP’s.

11.5 SEVERE WEATHER

During Forecasted Severe Weather Events, Contractors will be expected to take all precautions necessary, and are required to secure or remove any loose items that may become projectiles, especially any material or equipment stored on elevated levels. Contractors will be required to perform the following when requested by the VA (as needed according to the type of severe weather event) -

• A survey of your areas to determine the potential exposure to high winds and flooding.

Identify areas in need of protection, material removal, etc.

• Examine structures or materials covered with tarps to ensure they will remain standing in a storm. The wind can fill a tarp and cause it to pull down materials or structures.

• Examine any materials stored in your laydown area and secure as needed. The Laydown area onsite may be prone to flooding, recommend anything of value to be removed.

• Dumpster removal.

• Porta-John removal.

• Scaffolding removal.

• Removal of any ladders, trailers, temporary structures, or other items that could become a projectile.

• Removal of any materials, supplies, or tools that are stored on rooftops.

• Removal of any materials identified by the VA that could become a hazard during severe weather.

11.5 KEY CONTROL

The General Contractor shall provide duplicate keys and lock combinations to the Resident Engineer for the purpose of security inspections of every area of project including tool boxes and parked machines and take any emergency action. The General Contractor shall turn over all permanent lock cylinders to the VA locksmith for permanent installation. See Section 08 71 00, DOOR HARDWARE and coordinate.

11.6 SECURITY PLAN

The General Contractor is responsible for assuring that all sub-contractors working on the project and their employees also comply with these regulations. General Contractor’s employees shall not enter the project site without appropriate badge. They may also be subject to inspection of their personal effects when entering or leaving the project site. For working outside the “regular hours” as defined in the contract, The General Contractor shall give 3 days’ notice to the Contracting Officer so that security an be provided for the employees. This notice is separate from any notices required for utility shutdown described later in this section. No photography of VA premises is allowed without written permission of the Contracting Officer. VA reserves the right to close down or shut down the project site and order General Contractor’s employees off the premises in the event of a national emergency.

The General Contractor may return to the site only with the written approval of the Contracting Officer.

11.7 MOTOR VEHICLE RESTRITIONS

Vehicle authorization request shall be required for any vehicle entering the site and such request shall be submitted 24 hours before the date and time of access. Access shall be restricted to picking up and dropping off materials and supplies.

Separate permits shall be issued for General Contractor and its employees for parking in designated areas only.

11.7 FIRE SAFETY

Fire Safety Plan: Establish and maintain a fire protection program in accordance with 29 CFR 1926.

Prior to start of work, prepare a plan detailing project-specific fire safety measures, including periodic status reports, and submit to Facility Safety Manager for review for compliance with contract requirements in accordance with Section 01 33 23, SHOP DRAWINGS, PRODUCT DATA AND SAMPLES Prior to any worker for the contractor or subcontractors beginning work, they shall undergo a safety briefing provided by the general contractor’s competent person per OSHA requirements. This briefing shall include information on the construction limits, VAMC safety guidelines, means of egress, break areas, work hours, locations of restrooms, use of VAMC equipment, etc. Documentation shall be provided to the Resident Engineer that individuals have undergone contractor’s safety briefing.

11.8 INFECTION PREVENTION MEASURES

Implement the requirements of VAMC’s Infection Control Risk Assessment (ICRA) team. ICRA Group may monitor dust in the vicinity of the construction work and require the Contractor to take corrective action immediately if the safe levels are exceeded. Establish and maintain a dust control program as part of the contractor’s infection preventive measures in accordance with the guidelines provided by ICRA Group. Prior to start of work, prepare a plan detailing project-specific dust protection measures, including periodic status reports, and submit to Resident Engineer and Facility ICRA team for review for compliance with contract requirements in accordance with Section 01 33 23, SHOP DRAWINGS, PRODUCT DATA AND SAMPLES. All personnel involved in the construction or renovation activity shall be educated and trained in infection prevention measures established by the medical center.

11.9 PROTECTION OF EXISTING VEGETATIONS, STRUCTURES, EQUIPMENT,

UTILITIES, AND IMPROVEMENTS

The Contractor shall preserve and protect all structures, equipment, and vegetation (such as trees, shrubs, and grass) on or adjacent to the work site, which are not to be removed and which do not unreasonably interfere with the work required under this contract. The Contractor shall only remove trees when specifically authorized to do so and shall avoid damaging vegetation that will remain in place. If any limbs or branches of trees are broken during contract performance, or by the careless operation of equipment, or by workmen, the Contractor shall trim those limbs or branches with a clean cut and paint the cut with a tree-pruning compound as directed by the Contracting Officer. The Contractor shall protect from damage all existing improvements and utilities at or near the work site and on adjacent property of a third party, the locations of which are made known to or should be known by the Contractor. The Contractor shall repair any damage to those facilities, including those that are the property of a third party, resulting from failure to comply with the requirements of this contract or failure to exercise reasonable care in performing the work. If the Contractor fails or refuses to repair the damage promptly, the Contracting Officer may have the necessary work performed and charge the cost to the Contractor.

11.10 STORAGE SPACE FOR DEPARTMENT OF VETERAN AFFAIRS EQUIPMENT

(TO INCLUDE RELOCATED ITEMS)

Contractor shall disconnect, dismantle as necessary, remove, and reinstall in new location, all existing equipment and items indicated by symbol "R" or otherwise shown to be relocated by the Contractor.

Contractor shall coordinate with COR and provide appropriate space in building accessible from ground level without use of elevators for storage of certain materials and equipment by Department of Veterans Affairs. Provide such space with adequate light, ventilation and heat in season and lock for adequate security. Contractor shall also install and connect portion of nearest specified fire protection system including all apparatus for instant use to provide water for adequate fire protection of storage space.

12.0 PROJECT SCHEUDLE REQUIRMENTS

Show on the project schedule the sequence of work activities/events required for complete performance of all items of work. VA inspection and acceptance activity/event with a minimum duration of five workdays at the end of each phase and immediately preceding any VA move activity/event required by the contract phasing for that phase. The schedule shall be generally numbered in such a way to reflect either discipline, phase, or location of the work.

12.1 PROGRESS REPORTING

Monthly schedule update meetings will be held on dates mutually agreed to by the COR and the Contractor. Contractor and their CPM consultant (if applicable) shall attend all monthly schedule update meetings. The Contractor shall accurately update the Project Schedule and all other data required and provide this information to the COR three workdays in advance of the schedule update meeting. It is the responsibility of the Contractor to record and provide meeting minutes per schedule meeting.

12.2 CHANGES TO THE SCHEDULE

Within 30 calendar days after VA acceptance and approval of any updated project schedule, the Contractor shall submit a revised electronic file (s) and a list of any activity/event changes including predecessors and successors for any of the following reasons:

Delay in completion of any activity/event or group of activities/events, which may be involved with contract changes, strikes, unusual weather, and other delays will not relieve the Contractor from the requirements specified unless the conditions are shown on the CPM as the direct cause for delaying the project beyond the acceptable limits.

Delays in submittals, or deliveries, or work stoppage are encountered which make rescheduling of the work necessary.

The schedule does not represent the actual prosecution and progress of the project.

When there is, or has been, a substantial revision to the activity/event costs regardless of the cause for these revisions.
