About this file

This is a Conflict of Interest Affidavit form for the Maryland Department of Juvenile Services' solicitation for a Dental Assistant position at Baltimore City Juvenile Justice Center (BCJJC) and Cheltenham Youth Detention Center (CYDC). The affidavit, effective July 2024 and referenced under COMAR 21.05.08.08, must be submitted with all bids and proposals. The contract opportunity covers a 9-month period from January 1, 2026 through September 30, 2026, and applies to all offerors, contractors, consultants, subcontractors, and their employees or agents who will have authority to control or supervise any portion of the work.

The affidavit requires offerors to warrant that no relevant facts or circumstances exist that would create a conflict of interest, defined as situations where a person is unable to render impartial assistance due to other activities or relationships, or where objectivity in performing contract work is or could be impaired, or where an unfair competitive advantage exists. Offerors must disclose any potential conflicts in the designated section, and agree to immediately notify the procurement officer in writing of any actual or potential conflicts that arise after the affidavit is executed, including a detailed description of actions taken or proposed to mitigate or neutralize such conflicts. The affidavit must be executed under penalty of perjury by an authorized representative.
