About this file

This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to contractors entering into agreements with the county. The document requires contractors to certify their compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations and guidelines. The assurance is binding on the contractor, its successors, transferees, and assignees, and is required for contract awards following the date of the assurance.

The document contains two compliance pathways based on contractor size. Contractors employing fewer than 15 persons must certify compliance but are subject to an exception permitting them to refer disabled persons to other accessible service providers if facility modifications would constitute a significant alteration. Contractors employing 15 or more persons must designate a specific person to coordinate compliance efforts pursuant to 45 C.F.R. 84.7(a). The document requires the authorized official to provide contractor name, street address, city, state, zip code, the name of the designated 504 coordinator (if applicable), signature, title, and date. This assurance is a standard prerequisite document for the County of San Mateo's contract for Specialty Therapeutic Bed Rental Services at San Mateo Medical Center.
