About this file

This is an Assurance of Compliance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing Section 504 of the Rehabilitation Act of 1973 compliance requirements for contractors. The document requires all contractors to comply with Section 504 regulations, applicable Department of Health and Human Services (DHHS) regulations, and all related guidelines and interpretations. Contractors with 15 or more employees must designate a person to coordinate compliance efforts, while contractors with fewer than 15 employees are subject to an exception allowing referral of disabled persons to other accessible service providers if facility alterations would constitute a significant burden. The assurance is binding on the contractor, its successors, transferees, and assignees, and contractors must certify the accuracy of submitted information through an authorized official's signature.

The document serves as a mandatory compliance certification that contractors must execute as a condition of obtaining County contracts. By signing the assurance, contractors acknowledge their commitment to non-discrimination and accessibility standards under federal law. The form requires contractors to provide organizational information including the contractor's name, address, and the designated Section 504 compliance coordinator's name for applicable entities. This compliance requirement applies to the Pharmacy Consulting Services contract opportunity for San Mateo Medical Center, which is being procured by San Mateo County for a three-and-one-half-year term with proposals due February 13, 2026.
