About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, requiring contractor compliance with Section 504 of the Rehabilitation Act of 1973 and applicable Department of Health and Human Services (DHHS) regulations. The assurance applies to all contracts awarded after the document's issuance date and binds the contractor, its successors, transferees, and assignees. The document requires contractors to designate a 504 compliance coordinator if the organization employs 15 or more persons, or certifies that it employs fewer than 15 persons. The contractor must complete and submit the form with the designated 504 coordinator's name (if applicable), contractor name, street address or P.O. box, city, state, and zip code, along with the signature, title, and date of an authorized official.
The assurance document includes an exception provision under DHHS regulations allowing recipients with fewer than 15 employees to refer disabled persons to other accessible providers if facility accessibility compliance would require significant alterations to existing facilities. This compliance assurance is a standard requirement for the County of San Mateo's contracting process and must be executed by all prospective contractors as a condition of contract award. The form serves as a binding representation of the contractor's commitment to non-discrimination and accessibility in service delivery in accordance with federal disability rights legislation.
