About this file

This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to contractors seeking contracts with the county. The document requires contractors to certify their compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. The assurance is binding on the contractor, its successors, transferees, and assignees, and serves as a condition for obtaining contracts after the date of execution. Contractors must provide their name, authorized official signature, title, and date, along with their street address or P.O. Box and city, state, and zip code.

The document includes a bifurcated requirement based on contractor size: contractors employing fewer than 15 persons may check option (a) with no additional designation required, while contractors employing 15 or more persons must check option (b) and designate a specific person to coordinate compliance efforts with DHHS regulations, providing that individual's name. An exception is noted for contractors with fewer than 15 employees who determine that compliance with facility accessibility regulations would require significant alterations to existing facilities; such contractors may refer individuals seeking services to other accessible providers as an alternative to making substantial facility modifications.
