About this file
This is an Assurance of Compliance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing Section 504 of the Rehabilitation Act of 1973 compliance requirements for contractors. The document requires all contractors entering into agreements with San Mateo County to certify their compliance with Section 504 and applicable Department of Health and Human Services (DHHS) regulations. Contractors must complete the assurance form by providing organizational information and designating a Section 504 compliance coordinator if the organization employs 15 or more persons. Organizations with fewer than 15 employees are exempt from designating a specific coordinator, though they remain subject to Section 504 accessibility requirements and may refer individuals to alternative accessible service providers if compliance would require significant facility alterations. The assurance is binding on the contractor, its successors, transferees, and assignees, and must be signed by an authorized official.
The document does not address pricing terms, set-asides, incumbents, or funding sources, as it functions as a compliance certification rather than a procurement document. The assurance applies to all County of San Mateo contracts executed after the document's issuance date and serves as a prerequisite condition for contract award. No specific performance metrics, deliverables, or contract term information is provided within this compliance attachment. The form requires certification that submitted information is complete and accurate to the best of the signatory's knowledge.
