About this file
This is an Assurance of Compliance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing Section 504 of the Rehabilitation Act of 1973 compliance requirements for contractors. The document requires all contractors entering into agreements with the County to certify compliance with Section 504 regulations, applicable Department of Health and Human Services (DHHS) regulations, and all related guidelines and interpretations. Contractors must complete the assurance form by indicating whether they employ fewer than 15 persons or 15 or more persons. Organizations employing 15 or more employees are required to designate a specific Section 504 compliance coordinator and provide that individual's name, along with the contractor's name, address, and contact information. The assurance binds the contractor, its successors, transferees, and assignees, with an authorized official required to sign and certify the accuracy of the information provided.
The document includes an exception clause noting that contractors with fewer than 15 employees may refer individuals with disabilities to other accessible service providers if achieving facility accessibility compliance would require significant alterations to existing facilities. This assurance serves as a prerequisite for contract award and reflects the County's commitment to non-discrimination in service delivery and employment practices under federal disability law. No specific contract term, pricing, or award information is contained within this compliance document, as it functions as a standard contractual requirement applicable to the Provider Time Tracking, Invoicing and Approval RFP issued by San Mateo Medical Center.
