About this file

This is a Section 504 Rehabilitation Act Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013. The document serves as a contractual commitment binding the contractor, its successors, transferees, and assignees to comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. The assurance is required as a condition for obtaining contracts from the County of San Mateo and must be signed by an authorized official of the contracting entity. The document includes checkboxes distinguishing between contractors employing fewer than 15 persons and those employing 15 or more persons, with the latter required to designate a specific individual responsible for coordinating compliance efforts. For contractors with 15 or more employees, the document requests the name of the designated 504 coordinator along with standard contractor identification information including street address, city, state, and zip code.

The compliance framework includes an exception clause noting that contractors employing fewer than 15 employees may refer individuals with disabilities to other accessible service providers if facility modifications to achieve accessibility would constitute a significant alteration to existing facilities. The document requires certification that submitted information is complete and accurate, with signature, title, and date fields for the authorized official. This assurance document is a standard prerequisite compliance requirement for County of San Mateo contracting and does not specify monetary values, renewal terms, or performance metrics beyond the affirmative obligation to maintain Section 504 compliance throughout the contract period.
