About this file
This is an Assurance of Compliance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, addressing contractor obligations under Section 504 of the Rehabilitation Act of 1973, as amended. The document serves as a binding compliance assurance that contractors must execute to obtain County contracts. The assurance requires contractors to comply with Section 504 regulations, all applicable Department of Health and Human Services (DHHS) regulations, and related guidelines and interpretations. The document includes provisions for contractors with fewer than 15 employees and those with 15 or more employees, with the latter required to designate a Section 504 compliance coordinator. Contractors must provide complete information including the name of the designated 504 person, contractor name, street address or P.O. Box, and city, state, and zip code.
The compliance assurance is binding on the contractor, its successors, transferees, and assignees, and the authorized official executing the document certifies that all information provided is complete and correct to the best of their knowledge. The document notes an exception for recipients with fewer than 15 employees, allowing them to refer disabled persons seeking services to other accessible providers if facility accessibility compliance would require significant alterations to existing facilities. This assurance form represents a standard compliance requirement for the County of San Mateo and is applicable to the HRSA Service Area Competition Application Support procurement referenced in the related contract opportunity.
