About this file
This is a Section 504 Rehabilitation Act Compliance Assurance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to the County's procurement for automated dispensing cabinets at San Mateo Medical Center. The form establishes mandatory compliance requirements for all contractors bidding on County contracts, requiring them to adhere to Section 504 of the Rehabilitation Act of 1973 as amended, along with all applicable Department of Health and Human Services (DHHS) regulations and related guidelines. Contractors are required to certify their compliance status and, if employing 15 or more persons, must designate a specific individual to coordinate accessibility compliance efforts, providing that person's name, the contractor's name, street address or P.O. Box, and city, state, and zip code on the form.
The assurance document is binding on the contractor, its successors, transferees, and assignees, with an authorized official required to sign and date the certification. Contractors employing fewer than 15 persons are exempt from the designation requirement but remain subject to Section 504 compliance obligations. The form notes that DHHS regulations provide an exception for small contractors (fewer than 15 employees) that may refer disabled persons seeking services to other accessible providers if compliance would require significant facility alterations. The contractor must certify that all information provided is complete and accurate, with the authorized official's title and signature required for validation of the compliance assurance.
