About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing regulatory requirements for contractors engaging with the county. The assurance obligates contractors to comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations, guidelines, and interpretations. The document applies to all contracts awarded after the date of execution and remains binding on the contractor, its successors, transferees, and assignees. Contractors must designate an authorized official to sign the assurance, certifying the accuracy of submitted information.
The document distinguishes compliance obligations based on organizational size. Contractors employing fewer than 15 persons follow one compliance track, while those employing 15 or more persons must designate a specific Section 504 compliance coordinator and provide that person's name and contact information. The regulation includes an exception allowing contractors with fewer than 15 employees to refer individuals with disabilities to other accessible service providers if facility modifications would constitute a significant alteration. All signatories must complete fields identifying the 504 compliance coordinator (if applicable), contractor name, street address or P.O. Box, city, state, and zip code, along with the authorized official's signature, title, and date of execution.
