About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to contractors seeking contracts with San Mateo County. The document serves as a binding assurance form that contractors must execute to demonstrate compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations and guidelines. Contractors are required to certify their commitment to these accessibility and non-discrimination requirements as a condition of contract award. The form requires contractors to indicate their employee count—those employing fewer than 15 persons check option (a), while those employing 15 or more persons check option (b) and designate a Section 504 Compliance Coordinator. The assurance is binding on the contractor, its successors, transferees, and assignees, with authorized officials required to sign and date the certification.
The document includes a notable exception clause clarifying that DHHS regulations permit contractors with fewer than 15 employees to refer disabled persons seeking services to other accessible providers if facility alterations would constitute a significant burden. This compliance assurance must be completed with the contractor's name, the 504 Coordinator's name (if applicable), street address or P.O. Box, city, state, and zip code. The authorized official must certify that the information provided is complete and correct, sign the document, provide their title, and include the date of execution. This form is a mandatory compliance requirement for all County of San Mateo contracts and serves as documentation of the contractor's acknowledgment and agreement to maintain accessibility standards throughout the contract term.
