About this file
The document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, specifically Attachment I related to Section 504 of the Rehabilitation Act of 1973. This form is a standard compliance document that requires contractors to affirm their commitment to non-discrimination and accessibility for persons with disabilities, particularly those with 15 or more employees.
The form requires contractors to either confirm they employ fewer than 15 persons or, if employing 15 or more persons, designate a specific individual to coordinate efforts to comply with DHHS regulations. The document includes provisions that allow organizations with fewer than 15 employees to refer handicapped individuals to alternative accessible service providers if significant facility modifications are not feasible. The form provides space for contractors to enter their organization's details, including name, address, and a designated 504 compliance coordinator, with a section for an authorized official's signature and certification.
