About this file

This document is an Assurance of Compliance with Section 504 of the Rehabilitation Act of 1973, issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, in connection with a Medical Physicist contract opportunity at San Mateo Medical Center (SMMC). The assurance form requires contractors to certify their compliance with Section 504 of the Rehabilitation Act and all applicable Department of Health and Human Services (DHHS) regulations. The form requires contractors to indicate whether they employ fewer than 15 persons or 15 or more persons, with contractors employing 15 or more persons required to designate a Section 504 coordinator responsible for coordinating compliance efforts. The contractor must provide their name, address, the name and title of the designated 504 person (if applicable), and the signature of an authorized official certifying the accuracy of the information provided.

This compliance assurance is binding on the contractor, its successors, transferees, and assignees, and must be completed as a condition of contract award. The document notes an exception for recipients with fewer than 15 employees, allowing them to refer individuals with disabilities to other accessible service providers if facility accessibility modifications would require significant alterations to existing facilities. No pricing terms, specific performance dates, or other contract-specific requirements are detailed in this compliance form, as it serves solely as a regulatory assurance document required for contract execution.
