About this file
This is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, addressing Section 504 compliance obligations for contractors entering into agreements with San Mateo County. The document requires contractors to certify their compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations and guidelines. Contractors must complete the form by indicating their employee count and, if they employ 15 or more persons, designating a Section 504 compliance coordinator. The assurance binds the contractor, its successors, transferees, and assignees, and the signatory must be an authorized official capable of representing the contractor.
The form includes a notable exception provision stating that contractors with fewer than 15 employees may refer individuals to other accessible service providers if complying with facility accessibility requirements would require significant alterations to existing facilities. This assurance is required as a condition of obtaining contracts from San Mateo County and must be completed and signed by an authorized representative with the date of execution. No specific pricing terms, funding amounts, or contract duration are addressed in this compliance document, as it serves solely as an attestation of the contractor's commitment to nondiscrimination requirements under federal law.
