About this file

This document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, establishing compliance requirements with Section 504 of the Rehabilitation Act of 1973. The form serves as a binding assurance that contractors entering into contracts with San Mateo County will comply with Section 504 regulations, applicable Department of Health and Human Services (DHHS) regulations, and all related guidelines and interpretations. Contractors are required to complete and sign the form as a condition of contract award, with the assurance binding on the contractor, its successors, transferees, and assignees.

The form contains two primary compliance pathways: contractors employing fewer than 15 persons may comply with limited accessibility requirements, with an exception allowing referral to alternative accessible providers if facility alterations would be prohibitively costly; contractors employing 15 or more persons must designate a Section 504 compliance coordinator and provide that individual's name and contact information. The authorized official must certify the accuracy of the information provided and sign the assurance, which becomes a material term of any resulting contract with the County of San Mateo for services, including the Material Handling Equipment Maintenance, Repair, and Operator Safety Certification Services being solicited.
