About this file
This is a Section 504 Compliance Assurance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to the laser equipment and technical support services procurement for San Mateo Medical Center's operating room. The document establishes mandatory compliance requirements for all contractors bidding on or awarded contracts with the County of San Mateo. Contractors must certify their commitment to comply with Section 504 of the Rehabilitation Act of 1973, as amended, and all applicable Department of Health and Human Services (DHHS) regulations. The assurance applies to all contracts issued after the date of execution and is binding on the contractor, its successors, transferees, and assignees. Contractors are required to designate a Section 504 compliance coordinator if the organization employs 15 or more persons; those with fewer than 15 employees are exempt from this requirement but may be required to refer disabled persons seeking services to accessible alternative providers if facility accessibility modifications would constitute significant alteration.
The form requires authorized officials to certify under penalty of perjury that all provided information is complete and accurate. Contractors must complete the document by entering the name and contact information of the designated 504 coordinator (if applicable), contractor name, street address, city, state, and zip code, along with the authorized official's signature, title, and date of execution. This compliance assurance is a prerequisite for contract award and reflects the County of San Mateo's commitment to nondiscrimination and accessibility standards in all procurement activities, ensuring that individuals with disabilities have equal access to services and employment opportunities related to county contracts.
