About this file

This document is an Attachment I form for Assurance of Compliance with Section 504 of the Rehabilitation Act of 1973, issued by the County of San Mateo Contract Compliance Committee on August 5, 2013. The form is a standard compliance document that requires contractors to affirm their commitment to accessibility regulations for individuals with disabilities. Contractors must indicate whether they employ fewer than 15 persons or 15 or more persons, and if the latter, they must designate a coordinator to ensure compliance with DHHS regulations.

The form provides an exception for recipients with fewer than 15 employees, allowing them to refer individuals with disabilities to alternative accessible service providers if making significant facility alterations is not feasible. The document is part of the County of San Mateo's procurement process, specifically related to a potential Freight Management Services contract for San Mateo Medical Center, though this specific contract is not detailed within this compliance form. The primary purpose of this attachment is to ensure that contractors receiving federal funds or working with government entities maintain appropriate accessibility standards for individuals with disabilities.
