About this file

This is a technical questionnaire attachment from a Request for Proposal (RFP) for Pharmacy Benefit Management (PBM) services issued by the Commonwealth of Kentucky Employees' Health Plan (KEHP). The RFP seeks a PBM vendor to provide retail, mail order, and specialty pharmacy benefits management services starting January 1, 2026 for an initial 36-month contract term. The questionnaire covers extensive technical requirements across multiple service areas including formulary management, retail pharmacy networks, specialty pharmacy services, mail order operations, customer service, implementation support, account management, claims processing, eligibility management, reporting capabilities, and performance guarantees.

The document outlines detailed performance metrics and service level agreements the vendor must meet, including requirements for claims accuracy, network access, customer service response times, implementation timelines, and reporting deliverables. It requires vendors to provide pass-through pricing for retail pharmacy claims and includes provisions for market checks to ensure pricing competitiveness. The questionnaire also addresses Kentucky-specific requirements such as compliance with "any willing provider" laws and mail-order drug options for maintenance medications. Notable financial requirements include performance guarantees with penalties, an audit fund of $350,000 per contract year, a $100,000 pre-implementation audit fund, and discount credits of $2,000,000 (Year 1), $1,500,000 (Year 2) and $1,000,000 (Year 3).
