About this file

This is a Responsibility Inquiry form (Attachment G) issued by the Oregon Department of Energy (ODOE) for RFQ 24-026 regarding a SEED Energy Analysts position through the Oregon Buys procurement system (Number S-33000-00010556). The form serves as a pre-award evaluation document to assess the financial stability, operational capacity, and legal standing of interested proposers before contract execution. Proposers must complete a comprehensive eight-question assessment addressing their available resources, previous contract performance, criminal and civil history, litigation exposure, financial health, bankruptcy status, business licensing, and pay equity certification compliance. The form requires proposers to certify the accuracy and completeness of their responses under penalty of misrepresentation, with an authorized representative signature and date required for submission.

The Responsibility Inquiry process allows ODOE to request additional documentation beyond the form itself, including profit-and-loss statements, balance sheets, cash flow analysis, assets-to-liabilities ratios, credit information, bonding capacity, insurance verification, personnel records, and performance history under previous contracts. Failure to promptly provide requested documentation or demonstrate responsibility may result in ODOE issuing a non-responsibility finding and contract rejection. The Pay Equity Certificate requirement applies only to proposers employing 50 or more full-time workers if the contract price exceeds $500,000, with an exemption for architectural, engineering, photogrammetric mapping, transportation planning, and land surveying services. Proposers who previously submitted a Pay Equity Certificate with their bid or proposal need not resubmit it with this form.
