About this file
This is an invasive plant inventory field form template designed for the USDA Forest Service to document invasive species infestations across surveyed areas. The form serves as a data collection instrument for the 2026 Mottet Botany Surveys on the Umatilla National Forest, which aims to conduct botanical field surveys for target species including vascular plants, non-vascular plants, and invasive species to inform forest management decisions and mitigation strategies.
The form requires examiners to record essential site information including a unique Site ID (formatted as District_SpeciesCode_SequentialNumber), survey date, land ownership designation (Forest Service, State, Private, or BLM), and examiner name. For each invasive species encountered, the form captures the species name, USDA Plants Code, distribution pattern (evenly distributed, grouped, linear, or patchy), and percent cover across seven defined coverage ranges from >0-1% to 95.1-100%. The GPS location must be recorded in UTM NAD 83 Zone 10 projection format, with coordinates preferably taken near the centroid of the infestation polygon. The form mandates that each inventory record be associated with a polygon shapefile displaying the full extent of infestation, and includes specific definitions distinguishing between Total Area (gross area encompassing the entire infestation), Percent Infested (portion of total area occupied by invasive species), and Infested Area (actual area occupied, calculated as the product of percent infested multiplied by total area).

GPS Location: X________________ Y_________________ Projection must be UTM NAD 83 Zone 10 Note: each inventory field form shall be associated with a polygon .shp displaying the extent of the infestation. This GPS location is therefore just one point within that polygon, preferably taken in the centroid region.

Ex.: 061406_CESTM_01 Total Area (Gross Area) - The area of encompassing infestation. Think of the area of represented by polygon (deformed circle on map) that contains the entire infestation. The polygon can be any shape (long and skinny, amoeboid, or nearly circular) as long at it encompasses the infestation. For a gravel pit it may represent edges of the gravel pit. This is the area of a blob on a map that indicates where the infestation is located.
Percent Infested – The portion of the Total Area occupied by the invasive species.
Infested Area- The area actually occupied by invasive species. It is the mathematical product of the Percent Infested multiplied by Total Area.
