About this file

This is a Past Performance Questionnaire for the Department of Veterans Affairs (VA) Strategic Acquisition Center's solicitation 36C10X26R0032 for NX EQ EEG Systems. The questionnaire is designed to be completed by organizations that have previously worked with offerors bidding on this contract. References are limited to a maximum of five from the past three years and must be submitted to Levi Russin at Levi.Russin@va.gov no later than July 29, 2026, at 12:00 PM ET.

The questionnaire evaluates offerors across four key performance categories using a three-point scale (1=Unsatisfactory, 2=Satisfactory, 3=Excellent): Quality of Service (compliance with contract requirements, accuracy of deliverables, personnel effectiveness, and technical excellence); Cost Control (cost efficiencies, effective cost management, and timely accurate invoicing); Timeliness of Performance (meeting milestones, reliability, responsiveness to technical direction); and Business Relations (contract management, cooperative behavior, responsiveness to requirements, and problem resolution). References are asked to provide ratings and comments for each category, indicate whether any show cause, stop work, or termination orders were issued on the contract, and state whether they would choose to work with the offeror again. The questionnaire requires basic reference information including name, title, contact details, organization name, contract period, total contract value, and a brief description of services or commodities provided.
