About this file

This is a Past Performance Questionnaire (PPQ) template used by the U.S. Department of Veterans Affairs to evaluate contractor performance on Radiation Safety Officer (RSO) services contracts at the Orlando VA Healthcare System.

The questionnaire is divided into five parts: Part I, completed by the offeror, captures company information (DUNS/UEI, CAGE code, address), point of contact details, contract specifics (number, title, value, period of performance, type), and facility/modality information (facility type, number of sites, radiation modalities overseen such as nuclear medicine, linear accelerator, CT simulator, fluoroscopy, diagnostic X-ray, mammography, and brachytherapy). Parts II through V are completed by the reference/evaluator and submitted directly to the Contracting Officer. Part II collects reference information including name, title, organization, phone, email, role during contract (Contracting Officer, COR, Program Manager, Radiation Safety Committee Chair, Chief of Nuclear Medicine, or other), and length of familiarity with the offeror's performance.

Part III requires performance ratings across seven sections using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory): Section A addresses quality of service (radiation safety program oversight, survey accuracy, ALARA adherence, shielding design, ACR/AAPM/TJC compliance, training effectiveness); Section B covers regulatory compliance including 10 CFR Parts 20 and 35, FDA requirements, OSHA compliance, NRC/state correspondence, and incident reporting; Section C evaluates schedule and deliverables (Quality Control Plan, committee reports, meeting minutes, program reviews, recordkeeping, inspections); Section D assesses key personnel qualifications and certifications; Section E examines emergency response capabilities; Section F evaluates business relations and management; and Section G reviews cost control and invoicing accuracy. Parts IV and V include narrative assessment questions regarding strengths, weaknesses, performance problems, schedule/budget adherence, terminations or notices, contract re-award recommendation, and overall performance rating, concluding with certification and Privacy Act notice.
