About this file
This document is an Attachment F - Certified Disadvantaged Business Outreach Plan related to a Request for Proposal (RFP) issued by the Oregon State Hospital (OSH) for a single award of goods and services to ensure the OSH Pharmacy meets compliancy with ORS 689.564 and other federal requirements. The RFP requests proposals from firms that can provide an entire solution including, but not limited to, printers, medication bottle labels, leaflets, training, and miscellaneous goods and services for medication bottles in dual, multiple languages. The RFP includes jurisdictions of Marion County and the City of Salem.
The document outlines requirements for offerors to certify if they are an Oregon certified firm and to provide lists of certified and non-certified firms they have had contractual relationships with in the last two years. Offerors must also describe the steps they will take to solicit certified firms for subcontracting opportunities and the subcontracting opportunities and approximate dollar values available if awarded the contract. Offerors must also be willing to report the identity of each subcontractor and the value of each subcontract to the Oregon Certification Office for Business Inclusion and Diversity (COBID) if awarded the contract.
