This is a sample personal/professional services contract template from the State of Oregon, Oregon Department of Human Services (ODHS), designed for provider navigator services across three distinct programs: Homecare Workers (Office of Aging and People with Disabilities), Personal Care Attendants (Oregon Health Authority - Medicaid Division), and Personal Support Workers (Office of Developmental Disabilities Services). The contract establishes a framework for contractors to provide navigation and support services to applicants and enrolled providers within designated service areas, with services to be delivered in a timely and person-centered manner. The contract becomes effective upon execution by all required parties and expires on March 31, 2025, with potential for extension up to a total of five years at ODHS's discretion. Contractors must acknowledge provider navigator referrals within three business days, attempt contact with applicants or enrolled providers within five business days, and complete Provider Navigator Plans for each participant within three business days. Services include assistance with enrollment paperwork, background checks, training registration, resume building, interviewing skills, professional development, credential tracking, computer literacy, and resource identification. Contractors are required to participate in all scheduled Agency trainings and meetings and must maintain a backup provider navigator to ensure service continuity.
Compensation is structured at $63.79 per hour for all direct services provided, including individualized training, group training sessions, travel time, and appointment scheduling, with a maximum of 20 hours per individual unless additional hours are approved by the contract administrator. Billing and administrative services are compensated at $31.90 per hour with a maximum of four hours per month. Cancellation and no-show fees of $31.90 for in-person appointments and $15.95 for phone or virtual appointments are reimbursable, along with mileage at the IRS rate, postage, training materials, and approved interpreter services. Contractors must submit monthly billing reports by the 10th of the month following service delivery, including detailed statistical information on referrals, service areas, applicant and provider demographics, and program outcomes. The contract requires contractors to maintain comprehensive insurance coverage including workers' compensation, commercial general liability ($1,000,000 per occurrence), automobile liability ($1,000,000 combined single limit), and professional liability ($1,000,000 per claim). Contractors must comply with all federal and state laws, including HIPAA requirements, the Oregon False Claims Act, nondiscrimination provisions, mandatory abuse reporting statutes, and federal regulations pertaining to Medicaid services. The contract is governed by Oregon law and subject to termination by ODHS for convenience with 30 days' notice or immediately for cause, default, or failure to receive adequate funding.
