About this file

This document is a Federal Debarment Form (Attachment E) for RFP No. ILF0020, likely associated with a state or local government procurement process. The form is a certification document requiring vendors to disclose their legal and regulatory compliance status, specifically addressing potential exclusions, criminal convictions, pending indictments, and past transaction terminations related to federal, state, or local governmental transactions.

The certification requires vendors to attest that they and their principals have not been excluded, disqualified, convicted of specific offenses within the past three years, or terminated from public transactions. Vendors must provide their company name, an authorized representative's details, signature, and a Unique Entity Identifier (UEI) Number. The form emphasizes that false or erroneous certifications may result in potential suspension, debarment, or other remedies pursued by the federal government. The document appears to be a standard compliance requirement for entering into governmental transactions, with a revision date of 5/9/22.
