About this file
The document is a Federal Debarment Certification form (Attachment E) for RFP No. ILF0012, associated with a government procurement process. The form requires bidders to certify their legal and ethical standing by confirming they are not presently excluded, disqualified, or involved in criminal proceedings that would prohibit them from participating in government transactions.
The certification specifically requires vendors to attest that they have not been convicted of specified offenses within the preceding three years, have not been criminally or civilly charged by governmental entities, and have not had public transactions terminated for cause. Bidders must provide their company/organization name, an authorized representative's details, signature, and unique entity identifier (UEI) number. The document emphasizes that providing false information could result in suspension or debarment from future government contracts, with the potential for additional remedies pursued by the federal government.
