About this file
This document is an Attachment D - Certification Regarding Debarment, Suspension, and Other Responsibility Matters for a Request for Bid (RFB) solicitation issued by the Wisconsin Department of Workforce Development (DWD) on behalf of the Northwest Wisconsin Workforce Investment Board (NWWIB). The RFB is seeking proposals to retain a One-Stop Operator to coordinate the service delivery of required one-stop partners and service providers in the Northwest Wisconsin region. The contract resulting from this solicitation will be a single award executed and administered by either NWWIB or another entity on behalf of NWWIB. DWD does not guarantee any specific quantity or dollar amount of services to be purchased.
The certification form requires the applicant to attest that neither the applicant nor its principals are presently excluded or disqualified, have been convicted of certain offenses, or have had public transactions terminated for cause or default within the preceding three years. Failure to provide an accurate certification may result in suspension or debarment remedies available to the federal government.
