About this file

This is a Past Performance Questionnaire template for Solicitation RFQ-47QACA26R0306 for Risk Management, Financial Analysis and Administrative Support Services. The document establishes evaluation procedures and rating criteria for assessing contractor past performance on comparable contracts.

Offerors must submit information on a minimum of three contracts performed within the last five years that are comparable in scope and complexity to the current requirement. For each reference contract, offerors provide contract details including project title, description of services performed, prime or subcontractor status, contract number, period of performance, contract type, contract value, current status, and key personnel. Offerors also furnish their company information (point of contact, address, DUNS, phone, email, GSA Schedule if applicable) and government agency contact information for both the Contracting Officer's Representative and Contracting Officer. Offerors forward the completed questionnaire to their past performance references and request clients complete the evaluation forms and submit them via email to bertha.russell@gsa.gov with the subject line identifying the solicitation number and evaluated company name.

The evaluation uses standardized performance rating criteria across eight performance elements: Quality of Technical Approach, Understanding of Requirements, Effective and Efficient Use of Resources, Timeliness of Performance, Quality of Performance/Customer Satisfaction, Business Behavior, Communication, and Cost Control. Each element is rated as Outstanding, Good, Acceptable, Marginal, Unsatisfactory, or Other (N/A or no data). The questionnaire includes a yes/no question regarding willingness to do business with the contractor again. A supplemental comment form allows evaluators to provide additional context supporting their ratings.
