About this file

This is a Past Performance Questionnaire for VA solicitation 36C25626Q1300 issued by the Michael E. Debakey VA Medical Center. The form is designed to collect reference information from previous customers or contracting officers regarding an offeror's past performance on related contracts. Responses must be submitted by email to Steven.berkeley@va.gov by September 14, 2026 at 4:00 PM CST, with the solicitation number included on the subject line. The questionnaire is confidential under Federal Acquisition Regulation requirements, and the names of evaluators providing reference information cannot be released.

The form evaluates offeror performance across five core rating categories using a five-point scale (with a "Not Applicable" option): overall quality and satisfaction, delivery performance, quality of service, problem resolution capability, and quality of contractor personnel. Additionally, the questionnaire includes yes/no questions regarding cure notices or corrective action letters issued in the past three years, contractor terminations for default, willingness to award another contract, discussion of adverse performance issues, database filing of past performance information, and exercise of contract options. Respondents must also provide contract dates, dollar value, and a brief description of services performed. The form permits attachment of past performance database reports or other supporting documentation deemed relevant to the evaluation.
