About this file

This is a State Corporation Commission (SCC) Form attachment for a Virginia Department of Labor and Industry contract opportunity related to medical examinations for VOSH and Boiler employees. The form establishes SCC registration requirements and compliance documentation that bidders must submit as part of their proposal. Bidders must indicate their business entity status, provide their SCC identification number if applicable, or demonstrate exemption from Virginia business registration requirements through one of several specified options. The document outlines Virginia's electronic procurement procedures, including bid receipt deadlines, public opening protocols, and late bid rejection policies. Additionally, the form includes conflict-of-interest certification requirements, whereby bidders must attest that they have not received compensation from any interested firm and that no immediate family members have received compensation exceeding $5,000 from interested firms.

The form serves as a compliance and conflict-of-interest verification mechanism within Virginia's procurement framework. Bidders that are out-of-state business entities may qualify for exemption if they do not regularly maintain employees, agents, offices, facilities, or inventories in Virginia, or may submit legal counsel opinions detailing their Virginia contacts and explaining why such contacts do not constitute transacting business in Virginia. Bidders with pending SCC applications may request a waiver to submit their SCC identification number after the bid due date, subject to Commonwealth discretion. The document references surplus property donation provisions for computers and related equipment to public schools and qualified charitable organizations, though this provision does not appear directly applicable to the medical examination services contract.
