About this file

This is a Site Visit Validation Form for Request for Bid (RFB) 758 2400000178 issued by the Department of Military Affairs. The form requires all bidders to conduct a mandatory site visit at Fort Knox, located at 9387 N. Wilson Road, Fort Knox, Kentucky 40121, between the hours of 9:00 a.m. and 3:00 p.m., Monday through Friday. Prospective bidders must contact Christopher Kehl at 502-694-0970 to schedule their site visit. The completed and signed Site Visit Validation Form must be submitted with the bid response to be considered for award. Bidders are responsible for inspecting the site, verifying actual square footage, and satisfying themselves regarding all general and local conditions that may affect contract performance costs. Failure to tour the site and obtain the required signature from an authorized Department of Military Affairs representative shall result in automatic disqualification of the bid response.

The form explicitly prohibits the disclosure of any pricing information to agency representatives during the site visit, and violation of this requirement renders the bid non-responsive and ineligible for award consideration. The agency representative must verify on the form that the bidder has toured all areas of the site mentioned in the solicitation and confirm that no pricing information was provided. Bidders are required to complete signature blocks for both the authorized Department of Military Affairs representative and the company representative conducting the site tour, along with printed names and dates. A failure to inspect the site does not constitute grounds for claims after contract award.
