About this file

This document provides instructions for completing an attachment to a federal grant application for the Homeless Veterans' Reintegration Program. Applicants must submit a Chart of Past Performance using data from a previously completed grant similar in size and scope to the HVRP. The chart requires information on the past grant including objectives, population served, funding amount, and performance goals and outcomes. Applicants must report on indicators such as participants served, placements into employment/training, retention rates, and earnings. Goals and outcomes for each indicator are required and used to award points. The HVRP is a competitive federal grant program through the Department of Labor that provides employment services to help veterans experiencing homelessness obtain meaningful jobs. Eligible applicants include state and local governments as well as non-profit organizations.

U.S. Department of Labor Veterans’ Employment and Training Service Chart of Past Performance

Instructions All applicants must fully complete Attachment C – Chart of Past Performance with performance from one previously completed grant that was similar in size, scope, and relevance to the requested grant. (Note: Completion of one Program Year[footnoteRef:1] is sufficient.) The grant can be a federally or non-federally funded grant or cooperative agreement, but not a contract. [1: A Program Year is defined as July 1 – June 30.]

Applicants should use performance indicators most similar to the following:

1. Participants Served/Enrolled

2. Placement into Employment/Training

3. Employment/Training Retention

4. Average or median earnings of participants four to six months after exiting the program VETS views the above indicators as the most critical to demonstrating that the applicant’s past success in a similar program has prepared its organization to succeed in operating an HVRP project. Applicants may substitute a different indicator if it is applicable to the outcomes required in this FOA. DOL reserves the right to disqualify indicators that are not sufficiently similar to the indicators above and award zero points for missing and/or non-qualifying indicators. Each indicator is worth up to 5 points for a total of 20 points.

The Chart of Past Performance must include the grantor name, overall objectives of the grant, the number of participants served, the type of population served, funding amount, and performance goals and outcomes. The chart must demonstrate if and how the applicant successfully completed and managed the grant agreement. The chart must clearly identify the indicators used, a definition for how each outcome is calculated (e.g., the numerator and denominator for the outcome), and the percentage of the goal achieved.

Complete the Chart of Past Performance on the following pages.

94/115
81.7%
The enrollment rate (81.7%) is the number of participants enrolled at the end of the grant’s period of performance (94) divided by the overall enrollment goal (115). An enrollment is an eligible individual that receives a service funded by the program. Under certain conditions, an individual participant may have more than one enrollment.
Percentage of participants that were placed into employment
66.0%
71.3%
71.3/66.0
108%
The total number of participants employed in one or more jobs at the end of the grant’s period of performance divided by the total number of participants exiting the program (61.3%). Each placed participant must also be exited in order to obtain credit on this indicator.
Employment rate of participants 6 months after exiting the program
54.0%
45.4%
45.4/54.0
84.1%
The percentage of program exiters who were in unsubsidized employment 6 months after exiting the program. The employment rate (45.4%) is calculated at the end of the grant’s period of performance by dividing the total number of exiters that were employed 6 months after exit by the total number of exiters.
Average or median earnings of participants 4 to 6 months after exiting the program.
$7,000
$8,216
8,216/7,000
117.4%
The median earnings of program participants in unsubsidized employment 4 to 6 months after exiting the program. Earnings are calculated based on one quarter’s worth of wages (13 weeks). The median earnings ($8,216) is found at the end of the grant’s period of performance by listing all of the exited participants’ earnings 4 to 6 months after exit from lowest value to highest value and picking the middle value. If there are two values in the middle, the median is the average of those two values.
