About this file

This is an attachment document for the Commonwealth of Kentucky outlining required affidavits and attestations for bidders, offerors, and contractors responding to RFP 758 2700000070. The document does not specify particular products, services, quantities, response dates, or project details. Instead, it serves as a compliance and eligibility verification document that must be completed and notarized by all prospective contractors seeking to be awarded a contract or renew an existing contract with the state.

The affidavit document contains three distinct attestation sections. The first requires contractors to swear under penalty of perjury that they have not knowingly violated Kentucky's campaign finance laws and that contract award will not violate any campaign finance provisions. The second section is for bidders claiming resident bidder status, requiring attestation that the entity is authorized to transact business in Kentucky and has maintained Kentucky corporate income tax filings, unemployment insurance fund payments, and workers' compensation insurance for the year preceding contract advertisement. The third section applies to bidders claiming qualified bidder status, requiring confirmation that the entity and all subcontractors meet qualifications under 200 KAR 5:410(3) and will maintain compliance throughout the contract term. Each section requires execution by an authorized representative and notarization. The bidding agency reserves the right to request documentation supporting any status claims, and failure to provide such documentation may result in bidder disqualification or contract termination.
