About this file
This document is an affidavit form required by the Commonwealth of Kentucky for bidders, offerors, and contractors seeking to be awarded state contracts under RFB 758 2700000066. The affidavit serves as a compliance certification valid for one year from the date of execution and must be notarized by an authorized representative of the contracting party. The form contains three distinct affidavit sections that bidders may be required to complete depending on the specific contract requirements: a general campaign finance compliance affidavit, a resident bidder status affidavit, and a qualified bidder status affidavit.
The resident bidder affidavit requires bidders to certify that they are authorized to transact business in Kentucky and have maintained Kentucky corporate income tax filings, unemployment insurance fund payments, and an active workers' compensation policy for at least one year prior to and through the contract advertisement date. The qualified bidder affidavit applies to nonprofit corporations furthering KRS Chapter 163 purposes or qualified nonprofit agencies for individuals with severe disabilities that employ individuals with severe disabilities for not less than seventy-five percent of direct labor man hours. The Commonwealth reserves the right to request supporting documentation for any claimed status and may disqualify bidders or terminate contracts for failure to provide such documentation upon request. All affidavits require notarization and must be sworn under penalty of perjury.
